Anatomy of the Trigeminal Nerve (CN V1)
Anatomy
1. Overview
CLINICAL ANALOGY - The Sentinel Nerve of the Eye
- Imagine the eye as a VIP housed in a fortified chamber.
- The ophthalmic division of the trigeminal nerve (cranial nerve V1) is its dedicated security network - wiring the cornea, conjunctiva, upper eyelid, and forehead.
- structure (cornea, conjunctiva, lid skin, forehead) to a central alarm panel (trigeminal ganglion).
- The moment anything threatens the eye - a foreign body, a drop of acid, or an inflammatory process - V1 triggers the alarm: reflex tearing, blink, and pain.
- When this nerve is silenced (neuroparalytic keratitis, herpes zoster), the eye loses its guardian, and catastrophic surface breakdown follows undetected.
- Understanding V1 anatomy is therefore the foundation of understanding corneal sensation, reflex arcs, referred pain syndromes, and the anatomical logic behind superior orbital fissure (SOF) and cavernous sinus lesions.
2. Overview of the Trigeminal Nerve
| Parameter | Detail |
|---|---|
| Cranial nerve number | Cranial nerve V - largest cranial nerve |
| Functional components | • General somatic afferent (GSA) - primary function • Special visceral efferent (SVE) - motor to masticatory muscles (mandibular division [V3] only) |
| Divisions | • Ophthalmic division (V1) • Maxillary division (V2) • Mandibular division (V3) |
| Ganglion | • Trigeminal (Gasserian / Semilunar) ganglion - in Meckel's cave on petrous apex • Trigeminal cistern lies within Meckel's cave |
| Nucleus | • Principal sensory nucleus (pons) - fine touch • Spinal trigeminal nucleus (caudal pons and medulla) - pain & temperature • Mesencephalic nucleus - proprioception |
| Exit from skull (ophthalmic division [V1]) | Superior orbital fissure (SOF) |
| Ophthalmic division (V1) exits cranium through | Superior orbital fissure (SOF) • Nasociliary nerve within common tendinous ring • Frontal and lacrimal nerves lateral to it |
