Complicated cataract surgery
Cataract & Refractive Surgery
Key Points
- •Posterior capsule rupture is the most common serious intraoperative complication (1.5–4.5%) - its management is essential surgical knowledge.
- •Pseudoexfoliation is the most important risk factor for zonular dialysis (RR ~5–7x) and PCR (RR ~2–3x).
- •Tamsulosin causes IFIS in 40–90% of exposed patients - always ask about α1-blocker use pre-operatively. Stopping the drug does NOT prevent IFIS.
- •Never place a single-piece acrylic IOL in the sulcus - use a 3-piece IOL and reduce power by 0.5–1.0 D for sulcus placement.
- •For a dropped nucleus, close the case and refer urgently to a vitreoretinal surgeon for PPV - never attempt retrieval from the anterior approach.
