Phacolytic glaucoma
Cataract & Refractive Surgery
Key Points
- •Phacolytic glaucoma is an open-angle glaucoma caused by leakage of HMW lens proteins through an intact hypermature cataract capsule - the angle is open on gonioscopy
- •The anterior chamber is deep (not shallow) - this distinguishes it from phacomorphic glaucoma which has a shallow AC and closed angle
- •AC paracentesis showing protein-laden macrophages on Millipore filter is the diagnostic gold standard
- •Cataract extraction is curative - removing the offending lens eliminates the protein source and IOP normalises
- •Pre-operative IOP control with topical and systemic agents, followed by surgery within 24–48 hours, is the standard management approach
