Phakic IOL
Cataract & Refractive Surgery
Key Points
- •Phakic IOLs correct high refractive errors (myopia −6 to −20 D) by implanting a lens without removing the natural lens - preserving accommodation
- •The posterior chamber ICL (EVO ICL) is the current standard - placed in the sulcus behind the iris; made of biocompatible Collamer; foldable through a 3.0 mm incision
- •Vault (ICL-to-crystalline lens distance) is the most critical post-operative parameter: ideal 250–750 µm; low vault → cataract, high vault → glaucoma
- •The EVO ICL (V4c) has a central KS-AquaPORT (0.36 mm hole) that eliminates the need for pre-operative peripheral iridotomy
- •Key pre-operative requirements: ACD ≥3.0 mm (FDA), endothelial cell count age-dependent (2050–3875/mm²), stable refraction ≥1 year, age 21–60 (FDA, 2026), no cataract
