Traumatic cataract
Cataract & Refractive Surgery
Key Points
- •Vossius ring (iris pigment imprint on anterior lens capsule) is pathognomonic of blunt ocular trauma - always exclude angle recession, zonular damage, and retinal tears
- •Never use MRI when metallic IOFB is suspected - use CT orbit (non-contrast, thin cuts) to detect and localize the foreign body
- •Sunflower cataract (chalcosis lentis) = copper IOFB; siderosis lentis = iron IOFB (reduced ERG b-wave) - both require prompt IOFB removal to halt progressive toxicity
- •In pediatric traumatic cataract, early surgery within 1–2 weeks and aggressive amblyopia treatment are as important as the surgical technique
- •Angle recession glaucoma can present years to decades after blunt trauma - all patients require lifelong IOP monitoring after significant blunt injury
