Neurotrophic Keratitis

Cornea

1. Section 1

Introduction & Clinical Analogy

Exam pearl
  • Clinical Analogy: Imagine a sophisticated greenhouse where the glass panels (corneal epithelium) are maintained not just by water and sunlight, but by a hidden irrigation system of nerve signals (trigeminal trophic input). If you sever the irrigation pipes - the glass doesn't shatter immediately, but it slowly loses its self-repair ability, becomes fragile, cracks form (epithelial defects), and ultimately the entire structure collapses (stromal melt, perforation). Neurotrophic keratitis is exactly this: a cornea abandoned by its own nervous system, losing the trophic support needed for cellular survival, wound healing, and tear film stability.
  • Neurotrophic keratitis (NK) is a rare but devastating degenerative corneal disease caused by impaired trigeminal innervation. Unlike inflammatory corneal diseases, NK is fundamentally a disease of lost trophic support - the cornea is not attacked, it is simply neglected by the nervous system. This distinction drives every aspect of its management philosophy.

2. Definition & Epidemiology

Definition

  • Neurotrophic keratitis (also called neurotrophic keratopathy) is a degenerative corneal disease resulting from impaired trigeminal sensory innervation.
  • This leads to corneal anesthesia or hypoesthesia, which causes breakdown of corneal epithelial integrity and impaired tear film.
  • Neurotrophic keratitis can ultimately progress to stromal ulceration and perforation.

Epidemiology

ParameterDetail
Prevalence• fewer than 5 per 10,000 population (orphan disease)
Gender• No significant predilection
Age• Any age • older age is a risk factor
Laterality• May be unilateral or bilateral • systemic sensory neuropathies can cause neurotrophic keratitis
Orphan Disease Status• Cenegermin-bkbj (Oxervate) is a United States Food and Drug Administration (FDA)-designated orphan product for treatment of neurotrophic keratitis
