Simple Limbal Epithelial Transplantation (SLET)

Cornea

1. Clinical Analogy & Conceptual Understanding

ANALOGY
  • Think of the limbal stem cell niche as a "seedbank" at the border of a field. When a wildfire (chemical burn) destroys the crops (corneal epithelium) AND the seedbank, the field cannot regenerate on its own - weeds (conjunctival epithelium) invade. SLET is like transplanting a few seeds from a neighbouring healthy field (fellow eye) directly onto the burned soil (diseased cornea) using a biodegradable mat (amniotic membrane) as a temporary scaffold. The seeds self-organise, multiply, and repopulate the entire field within weeks.

2. What is SLET?

  • Simple Limbal Epithelial Transplantation (SLET) is a single-stage surgical procedure for the treatment of unilateral total Limbal Stem Cell Deficiency (LSCD).
  • A small biopsy (~2×2 mm) of healthy limbal tissue from the contralateral eye is transplanted onto an amniotic membrane (AM) on the diseased cornea.
  • The limbal tissue is divided into 8–10 small explants/pieces and spread across the surface.
LANDMARK REFERENCE
  • Simple Limbal Epithelial Transplantation (SLET) was first described by Virender S. Sangwan, Sayan Basu, and colleagues at the LV Prasad Eye Institute, Hyderabad, India, and published in the British Journal of Ophthalmology (2012).
  • Simple Limbal Epithelial Transplantation (SLET) revolutionized management of limbal stem cell deficiency (LSCD) by eliminating the need for laboratory culture, making the procedure accessible to centres without sophisticated cell culture infrastructure.
