Congenital glaucoma
Glaucoma
Key Points
- •PCG is caused by trabeculodysgenesis (arrested angle maturation) and the primary treatment is surgical - medical therapy alone is insufficient (AAO PPP Childhood Glaucoma 2023)
- •Classic triad: epiphora, photophobia, blepharospasm - often misdiagnosed as nasolacrimal duct obstruction
- •Haab's striae (horizontal Descemet's breaks) are pathognomonic - distinguish from vertical forceps injury tears
- •Brimonidine is CONTRAINDICATED in children <2 years (CNS depression, apnea, coma); use timolol 0.25% for medical temporizing
- •Optic disc cupping in infants may be partially reversible with IOP reduction - unlike adults; the most elastic lamina cribrosa allows recovery
- •CYP1B1 gene mutation is the most common identified genetic cause (15–30% globally, up to 100% in select populations)
