Malignant glaucoma
Glaucoma
Key Points
- •Malignant glaucoma presents with the classic triad: flat/shallow AC + elevated IOP + patent iridotomy - the key is that conventional treatments (LPI, miotics) do NOT work
- •Aqueous is misdirected posteriorly into/behind the vitreous - NOT a pupillary block mechanism. Miotics (pilocarpine) WORSEN the condition
- •First-line treatment: Atropine 1% QID (posteriorly rotates ciliary body) + aqueous suppressants + osmotics - successful in ~50% of cases
- •B-scan ultrasound is critical to rule out suprachoroidal hemorrhage/effusion (which also causes flat AC + high IOP but shows choroidal detachment)
- •Risk in the fellow eye is ~50% - prophylactic atropine and avoidance of miotics are essential when planning surgery
