Normal tension glaucoma
Glaucoma
Key Points
- •NTG = glaucomatous optic neuropathy with IOP consistently ≤21 mmHg; accounts for 30–40% of OAG in the West and up to 90% in Japan (Tajimi Study)
- •CNTGS proved 30% IOP reduction reduces progression from 35% to 12% - IOP lowering is still beneficial despite normal baseline
- •Disc hemorrhages are the strongest predictor of progression and are 2–3x more common in NTG than POAG
- •MRI brain/orbits is mandatory to exclude compressive optic neuropathy (pituitary tumor), especially if age <50, pallor > cupping, or vertical midline-respecting field loss
- •10-2 visual fields are essential - NTG threatens fixation with paracentral scotomas that may be missed on 24-2 alone
