Plateau iris syndrome
Glaucoma
Key Points
- •Plateau iris syndrome is defined by persistent angle closure DESPITE a patent LPI - the angle is narrowed by an anteriorly positioned ciliary body, NOT by pupillary block
- •Central AC depth is NORMAL (unlike pupillary block where AC is uniformly shallow) - this is the key clinical clue
- •UBM is the gold standard for confirming the diagnosis - shows anteriorly positioned/rotated ciliary processes and absent ciliary sulcus
- •Argon laser peripheral iridoplasty (ALPI) is the treatment of choice - contracts peripheral iris stroma, pulling it away from the TM
- •Unlike phacomorphic glaucoma, pilocarpine IS useful in plateau iris syndrome (pulls iris root away from the angle)
