Birth trauma to the eye
Ocular Trauma
Key Points
- •Descemet membrane tears from birth trauma are vertical or oblique - distinguishing them from congenital glaucoma tears which are horizontal/circumferential
- •IOP and corneal diameter are NORMAL in birth trauma and elevated/enlarged in congenital glaucoma - the critical differentiating features
- •Retinal haemorrhages occur in 20–40% of vaginal deliveries and resolve spontaneously within 2–4 weeks; persistence beyond 6 weeks warrants evaluation for non-accidental injury
- •Amblyopia prevention is the most important long-term management priority for persistent corneal opacity from Descemet tears
- •Forceps delivery accounts for 90% of traumatic Descemet membrane tears in neonates
