Intraocular foreign body
Ocular Trauma
Key Points
- •MRI is ABSOLUTELY CONTRAINDICATED if metallic IOFB suspected; CT orbits (1 mm thin cuts) is the gold standard with >95% sensitivity for metallic FB
- •Iron causes siderosis (heterochromia, mydriasis, cataract, ERG b-wave reduction); copper >85% causes fulminant reaction; copper alloy <85% causes chalcosis (sunflower cataract, KF-like ring)
- •A transiently supranormal ERG b-wave is the earliest ERG change in siderosis; progressive b-wave reduction occurs later, so serial ERG is mandatory for monitoring
- •Start IV vancomycin + ceftazidime within 6 hours; surgical removal ideally within 24 hours to reduce endophthalmitis risk
- •Inert materials (glass, gold, silver, platinum) may be observed; organic materials (wood) mandate urgent removal due to high infection risk
