Open globe injury
Ocular Trauma
Key Points
- •BETT classification: open globe = full-thickness wound → rupture (blunt, inside-out) or laceration (sharp, outside-in: penetrating / perforating / IOFB)
- •NEVER apply pressure to a suspected open globe - rigid eye shield only; no tonometry, no patching, no forced duction
- •Start IV vancomycin + ceftazidime within 6 hours; primary surgical repair within 24 hours (ideally <12 hours)
- •OTS calculated at presentation is the single best predictor of final visual outcome - OTS 5 has 92% chance of ≥20/40
- •Sympathetic ophthalmia risk is 0.2–0.5%; risk increases if severely damaged eye not enucleated by 14 days; monitor fellow eye
