Radiation injury
Ocular Trauma
Key Points
- •Radiation retinopathy is clinically identical to diabetic retinopathy - microaneurysms, haemorrhages, CWS, macular oedema, NV - but confined to the radiation field with a latency of 6 months–3 years
- •Dose thresholds: cataract ≥2 Gy single/5 Gy fractionated; retinopathy ≥30–45 Gy; optic neuropathy ≥50 Gy; dry eye ≥30 Gy
- •Radiation cataract is characteristically posterior subcapsular (PSC) - reflecting damage to the actively dividing germinative zone of the lens epithelium
- •Diabetes mellitus is the most important synergistic risk factor for radiation retinopathy - diabetics may develop it at lower radiation doses
- •Intravitreal anti-VEGF is the first-line treatment for radiation maculopathy and proliferative radiation retinopathy
