Traumatic optic neuropathy
Ocular Trauma
Key Points
- •IONTS (Levin et al., 1999): NO clear benefit of IV methylprednisolone or optic canal decompression over observation - the single most commonly tested TON fact
- •Normal optic disc + RAPD + poor vision after trauma = TON until proven otherwise; disc pallor develops at 4–6 weeks
- •Intracanalicular segment is most vulnerable in indirect TON - rigid bony canal creates compartment syndrome when nerve swells
- •CRASH trial showed increased mortality with high-dose steroids in head injury - most centres now avoid methylprednisolone for TON
- •Initial VA is the strongest prognostic predictor - NLP at presentation has <10% chance of useful vision recovery
