Traumatic endophthalmitis
Ocular Trauma
Key Points
- •Traumatic endophthalmitis is an ophthalmic emergency - treat immediately with intravitreal vancomycin 1 mg + ceftazidime 2.25 mg on clinical suspicion without waiting for culture results
- •Bacillus cereus endophthalmitis is the most devastating form - can cause panophthalmitis and NLP within 12–24 hours; >70% lose the eye
- •Incidence is 2–10% of open globe injuries - highest with retained organic IOFB and delayed primary repair (>24 hours, risk 3-fold higher)
- •Unlike post-surgical endophthalmitis, systemic IV antibiotics are essential in traumatic endophthalmitis due to higher-virulence organisms and panophthalmitis risk
- •The EVS demonstrated immediate vitrectomy benefit for LP-vision endophthalmitis; in traumatic cases the threshold for vitrectomy is even lower
