Traumatic retinal detachment
Ocular Trauma
Key Points
- •Retinal dialysis (superonasal, at ora serrata) is the most common traumatic retinal break - always perform scleral indentation after any blunt trauma
- •Dialysis-related RD characteristically presents months to years after the original injury - always take a trauma history in delayed RD
- •Tobacco dust (Shafer sign) - pigment cells in the anterior vitreous - is pathognomonic of a retinal break and mandates urgent peripheral retinal examination
- •Scleral buckle is the preferred surgery for traumatic dialysis RD in young phakic patients; PPV + silicone oil for GRT, PVR, or penetrating trauma RD
- •PVR is the most common cause of surgical failure - risk factors include GRT, vitreous haemorrhage, multiple breaks, endophthalmitis, and choroidal detachment
