Capillary hemangioma

Oculoplasty

Key Points

  • •Capillary hemangioma is the most common benign orbital tumor of infancy; increases in size with crying (key differentiator from dermoid cyst); positive for GLUT-1 (pathognomonic)
  • •Triphasic course: rapid proliferation (0–12 months), then slow involution (50% by age 5, 70% by 7, 90% by 9)
  • •Oral propranolol (2–3 mg/kg/day) is the first-line treatment; mechanism: vasoconstriction + VEGF inhibition + apoptosis; watch for hypoglycaemia
  • •Amblyopia is the most important ophthalmic complication - from stimulus deprivation, astigmatism, or strabismus; aggressive patching therapy is essential
  • •Kasabach-Merritt phenomenon is NOT associated with infantile hemangioma - it occurs with kaposiform hemangioendothelioma (common exam trap)
