Cavernous hemangioma
Oculoplasty
Key Points
- •Cavernous hemangioma is the most common benign orbital tumor in adults; presents with slowly progressive, painless, axial proptosis in the 4th–5th decade
- •It is GLUT-1 negative - distinguishing it from infantile hemangioma (GLUT-1 positive); now classified as a slow-flow venous malformation
- •MRI: well-encapsulated intraconal mass with progressive "fill-in" enhancement pattern (peripheral to central) - pathognomonic
- •Lateral orbitotomy is the standard surgical approach; the well-defined capsule allows complete excision with cure rate >98%
- •Does NOT change with Valsalva (unlike orbital varix) and does NOT involute (unlike infantile hemangioma)
