Chalazion
Oculoplasty
Key Points
- •Chalazion is a chronic, sterile, lipogranulomatous inflammation - NOT an infection; differentiate from hordeolum (acute, infectious)
- •ALWAYS send tissue for histopathology after I&C in patients >40 years - sebaceous cell carcinoma masquerades as recurrent chalazion
- •Conservative management (warm compresses) resolves ~25–50% within 6 months; I&C is the definitive treatment with 78–92% cure rate
- •The I&C incision is made vertically on the tarsal conjunctival surface - perpendicular to lid margin to avoid cutting meibomian glands
- •Recurrence rate 25–50% - must treat underlying MGD/blepharitis to prevent recurrence
