Dermoid cyst
Oculoplasty
Key Points
- •Dermoid cysts are choristomas - the most common benign orbital tumor in children; they arise from ectodermal tissue trapped at embryonic suture lines
- •Superotemporal orbit (frontozygomatic suture) is the most common location (60–70%); firm, non-tender, non-compressible mass fixed to periosteum
- •CT shows a well-circumscribed, low-density (fat-containing) cystic lesion with smooth bony remodelling - NOT bony destruction
- •Complete excision with intact capsule is mandatory - rupture causes intense foreign body granulomatous inflammation and increases recurrence risk
- •Always exclude encephalocele before excising any nasal orbital mass in a child (positive impulse with crying, bony defect on CT)
