Eyelid tumors
Oculoplasty
Key Points
- •BCC is the most common eyelid malignancy (85–95%); lower eyelid is the most common site; Mohs micrographic surgery achieves 99% cure rate
- •Sebaceous gland carcinoma is the "great masquerader" - mimics chronic chalazion and blepharitis; any recurrent chalazion in the same location in an elderly patient must be biopsied
- •Oil Red O stain confirms SGC (must be performed on frozen section, not formalin-fixed tissue); SGC shows pagetoid spread requiring conjunctival map biopsies
- •SGC is associated with Muir-Torre syndrome (Lynch syndrome variant) - SGC + visceral malignancy; screen with colonoscopy and mismatch repair gene testing
- •Eyelid reconstruction ladder: <25% direct closure; 25–50% Tenzel flap; 50–75% Hughes flap (lower) or Cutler-Beard (upper); >75% free graft + flap
