Facial nerve palsy (eyelid)

Oculoplasty

Key Points

  • •Bell palsy accounts for 60–75% of facial nerve palsies; oral prednisolone within 72 hours is the most important treatment, improving complete recovery from 64% to 83% at 3 months
  • •Assess Bell phenomenon in every patient - those without it are at highest risk for exposure keratopathy and need aggressive corneal protection
  • •House-Brackmann grading is the standard classification: Grade I (normal) to Grade VI (total paralysis); Grade IV and above have incomplete eye closure
  • •Gold weight implant is the procedure of choice for permanent lagophthalmos - gravity-assisted lid closure using a pretarsal implant
  • •Distinguish UMN (forehead spared) from LMN (forehead involved) facial palsy - this determines the entire diagnostic pathway
