Lacrimal gland tumors

Oculoplasty

Key Points

  • •The "Rule of 50s": 50% of lacrimal gland masses are epithelial; of these, 50% are benign (pleomorphic adenoma); of malignant epithelial tumors, the most common is adenoid cystic carcinoma
  • •Incisional biopsy is CONTRAINDICATED for suspected pleomorphic adenoma - tumor seeding through the pseudocapsule leads to multilocular recurrence and malignant transformation risk
  • •Pain in a lacrimal gland mass = red flag for adenoid cystic carcinoma (perineural invasion of CN V1); painless slow growth = pleomorphic adenoma
  • •Adenoid cystic carcinoma: Swiss-cheese (cribriform) pattern most common; basaloid (solid) pattern worst prognosis; hallmark is perineural invasion
  • •CT distinguishes: smooth bony remodelling (pleomorphic adenoma) vs bony destruction (adenoid cystic carcinoma) vs moulding without bone changes (lymphoma)
