Orbital cellulitis

Oculoplasty

Key Points

  • •PROPTOSIS is the key sign differentiating orbital cellulitis from preseptal cellulitis - along with restricted motility, decreased VA, and RAPD
  • •CT orbits with contrast is the GOLD STANDARD - shows source (sinusitis), extent, abscess formation, and guides surgical planning
  • •Chandler classification (Stages I–V) guides management: Stages I–II = medical; Stages III–V = often surgical
  • •Children <9 years with small medial SPA from ethmoid sinusitis may be trialled on IV antibiotics 24–48h before deciding on drainage
  • •Cavernous sinus thrombosis is suggested by BILATERAL orbital signs with CN III/IV/VI palsies - a life-threatening emergency
