Idiopathic orbital inflammation
Oculoplasty
Key Points
- •IOI is a diagnosis of exclusion - must rule out thyroid eye disease, lymphoma, sarcoidosis, GPA, and IgG4-RD before labelling as 'idiopathic'
- •Dramatic response to corticosteroids within 24–48 hours is characteristic and serves as a diagnostic aid
- •Unlike thyroid eye disease, IOI involves the extraocular muscle tendon - a key imaging differentiator on CT/MRI
- •Up to 25–50% of previously labelled IOI cases may actually represent IgG4-related orbital disease - always check serum IgG4
- •Sclerosing IOI has the worst prognosis and may require orbital radiotherapy or immunosuppression
