Orbital tumors
Oculoplasty
Key Points
- •NEVER incisionally biopsy a suspected pleomorphic adenoma - tumour seeding causes recurrence and risk of malignant transformation; must excise en bloc
- •A PAINFUL lacrimal gland mass is adenoid cystic carcinoma until proven otherwise - pain indicates perineural invasion, the hallmark
- •Rhabdomyosarcoma is the most common primary orbital malignancy in children - rapid proptosis with lid ecchymosis mimics trauma; treat with VAC chemo + radiation (>90% survival)
- •Cavernous haemangioma is the most common benign orbital tumour in adults - well-encapsulated, intraconal, progressive contrast enhancement on CT
- •Oral propranolol (1–3 mg/kg/day) is first-line for amblyogenic capillary haemangioma since 2008
