Subjective Refraction

Optics & Refraction

1. Overview

Clinical Analogy
  • Think of subjective refraction as fine-tuning a radio signal.
  • Objective methods (retinoscopy, autorefraction) get you close to the station - but there is still static.
  • Subjective refraction is the patient's hand on the dial, telling you the precise moment the signal becomes crystal clear.
  • You are using the patient's visual cortex as your endpoint detector - which is simultaneously your greatest asset and your greatest source of error.

2. Conceptual Framework

Definition & Goal

  • Subjective refraction is the determination of the refractive correction that provides the Best Corrected Visual Acuity (BCVA) using patient responses, typically following objective refraction as a starting point.
  • The optical goal is to find the lens combination that places the circle of least confusion (CoLC) on the fovea, minimising defocus from uncorrected astigmatism.

Key Optical Concepts

ConceptClinical Relevance
Far Point• The conjugate point of the retina with accommodation relaxed • the target of refraction
Circle of Least Confusion (CoLC)• In astigmatism, the dioptric midpoint between the two focal lines • where blur is minimised equally in all meridians
Sturm's Conoid• The 3D optical shape formed between the two focal lines in an astigmatic eye
Jackson Cross Cylinder (JCC)• Combined plus and minus cylinders of equal power and opposite sign • workhorse of cylinder refinement • available as ±0.25 dioptre cylinder or ±0.50 dioptre cylinder
