Angioid streaks
Retina & Vitreous
Key Points
- •Angioid streaks are bilateral crack-like breaks in a calcified, brittle Bruch membrane radiating from the peripapillary region - predisposing to CNV (~70–80% lifetime risk)
- •PEPSI mnemonic: Pseudoxanthoma elasticum (most common, ~85% have streaks), Ehlers-Danlos, Paget disease, Sickle cell, Idiopathic
- •Intravitreal anti-VEGF is first-line treatment for CNV; the streaks themselves have no treatment - counsel patients to avoid contact sports and eye trauma
- •Peau d'orange (granular mid-peripheral RPE mottling) and optic disc drusen are associated fundus findings; skin biopsy confirms PXE (elastic fibre calcification)
