Eales disease
Retina & Vitreous
Key Points
- •Eales disease is an idiopathic retinal periphlebitis affecting young males (90–95% male, age 20–40 years), most common in the Indian subcontinent
- •Diagnosis of exclusion - must rule out sarcoidosis, TB, syphilis, Behçet disease, and other vasculitides before diagnosis
- •Classic triad: peripheral periphlebitis → retinal non-perfusion → neovascularisation with vitreous haemorrhage
- •Strong association with TB hypersensitivity (Mantoux positive in ~50–70%); ATT commonly given in India alongside steroids
- •FFA is essential to delineate non-perfusion; PRP is the mainstay treatment for proliferative stage; vitrectomy for non-clearing vitreous haemorrhage
