Retinal detachment
Retina & Vitreous
Key Points
- •Rhegmatogenous RD is the most common type (~90%); always caused by a full-thickness retinal break allowing liquefied vitreous to enter the subretinal space
- •Macula-on RD is a surgical emergency - operate within 24 hours to preserve central vision; macula-off RD should be repaired within 7 days
- •Shafer sign (tobacco dust in anterior vitreous) is pathognomonic of a retinal break - always examine for tears in symptomatic PVD
- •Lincoff rules localise the primary break; myopia >-3D carries 10x risk; symptomatic PVD with vitreous haemorrhage has ~70% probability of a retinal tear
- •Final reattachment rate with multiple surgeries is >95%, but visual prognosis is determined by macula status at the time of surgery
