Retinal Vasculitis
Retina & Vitreous
Key Points
- •Retinal vasculitis: arteritis vs phlebitis (most common) vs capillaritis; infectious vs non-infectious; occlusive vs non-occlusive
- •Behcet: occlusive periarteritis, worst prognosis, hypopyon, oral/genital ulcers, HLA-B51; Sarcoidosis: 'candle-wax drippings', periphlebitis, ACE elevated; TB: periphlebitis with hemorrhages
- •FFA is the most important investigation: vessel wall staining (active), leakage (barrier breakdown), non-perfusion (ischemia); OCT for macular edema monitoring
- •CME is the most common cause of visual loss; treat with intravitreal anti-VEGF/dexamethasone implant; PRP for neovascularization from ischemia
- •Systemic workup: ACE + chest X-ray (sarcoidosis), Mantoux/IGRA (TB), ANCA (GPA), ANA (SLE), VDRL (syphilis), HLA-B51 (Behcet); treat underlying cause + immunosuppression
