Retina & Vitreous
Key Points
- •Gas tamponade: SF₆ (20% (18–20%) non-expansile, ~2 weeks) and C₃F₈ (14% non-expansile, ~6–8 weeks) are the main intraocular gases; ABSOLUTE CONTRAINDICATION: air travel and N₂O anaesthesia
- •Silicone oil: long-term tamponade (1000 or 5000 cSt); causes hypermetropic shift (+5 to +7 D); remove at 3–6 months; complications: emulsification, glaucoma, cataract, band keratopathy
- •PFCL: heavy liquid (SG ~1.76–2.0) for intraoperative retinal flattening only - NEVER left as long-term tamponade (retinal toxicity)
- •Heavy silicone oil (Densiron/Oxane HD): for inferior retinal pathology; remove within 3 months
- •Choice depends on: break location (superior vs inferior), PVR severity, positioning compliance, phakic status, air travel plans, and need for prolonged tamponade
