Behcet disease uveitis
Uvea
Key Points
- •Behcet disease is a neutrophil-mediated vasculitis strongly associated with HLA-B51, presenting as explosive recurrent panuveitis with shifting hypopyon and retinal vasculitis
- •Diagnosis is clinical using the ICBD 2014 criteria (score >=4): oral ulcers (2), genital ulcers (2), ocular lesions (2), skin (1), pathergy (1), vascular (1), neuro (1)
- •Posterior segment disease (retinal vasculitis) requires systemic immunosuppression - steroids alone are never sufficient; azathioprine is first-line (Yazici 1990 trial)
- •Infliximab is the biologic of choice for acute sight-threatening retinal vasculitis; adalimumab (VISUAL trials) is first-line for chronic maintenance in non-infectious uveitis
- •Retinal vasculitis in Behcet involves both arteries and veins - distinguishes it from sarcoidosis (veins only, candle-wax drippings)
