CMV retinitis
Uvea
Key Points
- •CMV retinitis occurs almost exclusively at CD4 <50 cells/microL - the most common ocular opportunistic infection in AIDS with a classic brushfire/pizza-pie fundus pattern and minimal vitritis
- •HAART (immune reconstitution) is the most important long-term treatment; anti-CMV therapy controls disease but cure requires immune recovery
- •Induction: IV ganciclovir or oral valganciclovir for 2–3 weeks, then maintenance with oral valganciclovir until CD4 >=100 for >=3–6 months
- •Immune recovery uveitis (IRU) develops in 15–40% after HAART initiation - paradoxical vitritis, CMO, ERM despite CMV control
- •Retinal detachment is the most common vision-threatening complication (30% of eyes); managed with vitrectomy and silicone oil tamponade
