Complicated cataract
Uvea
Key Points
- •Polychromatic lustre (iridescent rainbow-coloured refractile dots) is pathognomonic of complicated cataract - not seen in age-related or steroid-induced PSC
- •Ensure ≥3 months of quiescence (no active AC cells) before cataract surgery - the single most important factor for surgical success (AAO PPP)
- •Hydrophobic acrylic in-the-bag PCIOL is the IOL of choice in uveitic eyes - avoid silicone IOLs due to higher surface deposits and inferior capsular biocompatibility
- •Post-operative steroid taper is 8–12 weeks minimum (much longer than routine cataract 4-week taper) to prevent rebound inflammation
- •PCO rates in uveitic eyes are 40–60% (vs 5–10% in routine cataract), often requiring Nd:YAG capsulotomy within 2–3 years
