Endophthalmitis

Uvea

Key Points

  • •Endophthalmitis is an ophthalmic emergency - the EVS trial is the landmark study guiding management of post-cataract endophthalmitis
  • •EVS key finding: if presenting VA is LP only, immediate PPV gives 3x better visual outcomes vs tap alone; if VA is HM or better, tap + inject is equivalent to PPV
  • •IV antibiotics provided NO benefit in the EVS - systemic antibiotics are NOT indicated for post-cataract endophthalmitis (but ARE indicated for endogenous and post-traumatic cases)
  • •Intravitreal vancomycin (1 mg) + ceftazidime (2.25 mg) is the standard antibiotic combination
  • •Povidone-iodine 5% pre-operatively and intracameral cefuroxime at surgery end are the most effective preventive measures (ESCRS study)
