HLA-B27 anterior uveitis
Uvea
Key Points
- •Fibrin in the anterior chamber is the hallmark of HLA-B27+ AAU - no other common form of anterior uveitis produces such dramatic fibrin formation
- •Alternating unilateral involvement (right eye one episode, left eye next) is highly suggestive of HLA-B27-associated uveitis
- •HLA-B27 is found in 50% of all AAU cases (relative risk ~100-fold for ankylosing spondylitis, markedly elevated for AAU)
- •Ankylosing spondylitis is present in 30–50% of HLA-B27+ AAU patients - always ask about back pain, morning stiffness, and joint symptoms
- •Adalimumab (anti-TNF) has the strongest evidence for steroid-sparing therapy in recurrent non-infectious uveitis (VISUAL I & II trials)
