Intermediate uveitis
Uvea
Key Points
- •Snowballs (inferior vitreous aggregates) and snowbanking (pars plana exudate) are the hallmark findings - always perform scleral depression to detect snowbanking
- •CME is the principal cause of visual morbidity (30–50%) and drives treatment decisions - OCT is the key monitoring tool
- •Pars planitis is a diagnosis of exclusion - the term should only be used when systemic workup is negative and snowbanking/snowballs are present
- •In any patient >50 years with 'intermediate uveitis' refractory to steroids, vitreous biopsy to exclude primary vitreoretinal lymphoma is mandatory (IL-10:IL-6 >1)
- •MRI brain should be ordered in all cases to rule out multiple sclerosis - especially in young women with bilateral disease
