Panuveitis
Uvea
Key Points
- •Panuveitis mandates a comprehensive systemic workup - the differential includes sight-threatening and life-threatening conditions (Behçet, lymphoma, endocarditis)
- •Always exclude infection (especially syphilis and TB) BEFORE starting systemic immunosuppression - misdiagnosis can be catastrophic
- •A shifting, non-layered hypopyon is virtually pathognomonic of Behçet disease - unlike infectious hypopyon, it is sterile
- •VKH and sympathetic ophthalmia share identical histopathology (granulomatous choroiditis with Dalen-Fuchs nodules) - the key distinction is history of penetrating trauma
- •Panuveitis has the worst visual prognosis among uveitis subtypes - early aggressive IMT is essential for chronic non-infectious causes
