Posterior uveitis

Uvea

Key Points

  • •Syphilis serology (FTA-ABS) must be ordered in EVERY case of posterior uveitis - syphilis is the 'great mimicker' (AAO PPP)
  • •Toxoplasma retinochoroiditis is the most common cause of posterior uveitis worldwide - classic headlight in fog (focal white lesion through dense vitritis)
  • •CMV retinitis has minimal vitritis ('quiet eye') while ARN has prominent vitritis - this key distinction helps differentiate the two necrotising retinitides
  • •ICGA is superior to FFA for imaging choroidal pathology - essential for birdshot, serpiginous, VKH, and TB choroidal granulomata
  • •Never start systemic corticosteroids for posterior uveitis without first ruling out infection - immunosuppression in untreated syphilis, TB, or HSV/VZV can be devastating
