Scleritis
Uvea
Key Points
- •The Watson-Hayreh classification divides scleritis into anterior (diffuse, nodular, necrotising with/without inflammation) and posterior types
- •Necrotising scleritis carries 25% 5-year mortality when associated with systemic vasculitis (GPA, RA) - requires urgent systemic immunosuppression
- •Phenylephrine test distinguishes scleritis (deep vessels do NOT blanch) from episcleritis (superficial vessels blanch)
- •~50% of scleritis patients have an underlying systemic disease - systemic workup (ANCA, RF, ANA, etc.) is mandatory
- •Scleritis requires systemic treatment (NSAIDs, oral steroids, immunosuppression) - topical drops alone are insufficient
