Tubercular uveitis

Uvea

Key Points

  • •TB uveitis causes 5–25% of uveitis in endemic regions; serpiginous-like choroiditis and choroidal tuberculoma are the most characteristic posterior patterns
  • •NEVER start corticosteroids alone for suspected TB uveitis - always cover with concurrent ATT first; steroids alone cause fulminant TB reactivation
  • •Serpiginous-like (TB) choroiditis starts away from the optic disc in a multifocal, non-contiguous pattern - unlike classic (autoimmune) serpiginous choroiditis which starts peripapillary
  • •QuantiFERON-TB Gold Plus is preferred over Mantoux (TST) in BCG-vaccinated populations; a normal chest X-ray does NOT exclude ocular TB (absent in 30–50% of cases)
  • •ICGA is the single most sensitive imaging modality for choroidal TB granulomata - detects subclinical lesions invisible on FFA or clinical exam and is best for monitoring treatment response
